On November 14, 2002, Aries Insurance Company ("Aries") consented to and was placed into liquidation by the Second Judicial Circuit Court in and for Leon County, Florida.  The Florida Department of Financial Services, Division of Rehabilitation and Liquidation, is the court-appointed Receiver of Aries.

Proof of claim forms have been mailed.  In order to be considered timely-filed, completed proof of claim and W9 forms for the Aries Receivership estate had to be postmarked by Friday, November 14, 2003.  If you believe that your proof of claim form should be considered timely-filed, you may submit supporting documentation to us with your request for the Receiver to accept your proof of claim form as timely-filed. With appropriate supporting documentation, Chapter 631, Florida Statutes also gives you the right to petition the Receivership court (Second Judicial Circuit, Tallahassee, Leon County, Florida, case number 02-CA-1128) to accept your claim as timely-filed.

It is the claimant's responsibility to notify the Receiver, in writing, if there is a change in their name or mailing address.  When corresponding with the Receiver, be sure to reference "Aries Insurance Company" and the "Receiver's Identification Number", which will allow the Receiver to associate your request with the correct claim and receivership.
