Receiver Proof of Claim Form Information - Aries Insurance Company

In order to be considered timely-filed, completed proof of claim and W9 forms for the Aries estate had to be postmarked by Friday, November 14, 2003.  If you believe that your proof of claim form should be considered timely-filed, you may submit supporting documentation to us with your request for the Receiver to accept your proof of claim form as timely-filed.  With appropriate supporting documentation, Chapter 631, Florida Statutes also gives you the right to petition the Receivership court (Second Judicial Circuit Court, Tallahassee, Leon County, Florida, case number 02-CA-1128) to accept your claim as timely-filed.

Can you verify receipt of my proof of claim and W9 forms?

We have received thousands of proof of claim and W9 forms, and we are processing them as quickly as possible.  If you sent your proof of claim form by certified mail, return receipt requested, or if it was sent via FedEx, Airborne Express, etc., you should save the delivery certification cards as proof of mailing.  No confirmations of receipt of individual proof of claim forms will be mailed. 

I received the proof of claim form after the filing deadline.  Can I file a claim?  Will it be considered late-filed?

Yes, you may file a proof of claim.  Any proof of claim form postmarked after November 14, 2003 will be processed as late-filed.  If you believe that your proof of claim form should be considered timely-filed, you may submit supporting documentation to us with your request for the Receiver to accept your proof of claim form as timely-filed.  With appropriate supporting documentation, Florida Statute 631 gives you the right to petition the Receivership court (Second Judicial Circuit Court, Tallahassee, Leon County, Florida, case number 02-CA-1128) to accept your claim as timely-filed.

Will the deadline be extended?

No.

How do I obtain a proof of claim form?

If you would like a proof of claim form,  please contact the Receiver by using our "Contact Us" form. Alternatively, you may contact the Receiver at 800-882-3054.

I have been unable to download the proof of claim form from the website. What do I do now?

Please double-check your spelling.  Callers who have had this problem are misspelling “Aries” or “Receiver” or are adding spaces where none are needed.  If you are not filing a claim on behalf of a Premium Finance Company, click on “All Other Claimants.”  The instructions for completing the Proof of Claim form, the W9, and W9 completion-instructions are also available for downloading separately, as is the latest version of Adobe Acrobat Reader.  Most public libraries have internet access and their personnel can assist you in downloading the forms.
