LICENSED ENTITIES – A CHANGE IN BUSINESS LOCATION, OWNERSHIP OR A SUBSTANTIAL MODIFICATION TO THE PHYSICAL PREMISES OF A LICENSED ENTITY
A change of actual physical location, a change in ownership or a substantial modification to the physical premises of a licensed funeral establishment, cinerator facility, direct disposal establishment or centralized embalming facility requires Board approval. The licensed entity is required to submit the change for Board approval using the appropriate entity license type application form.

NAME CHANGE
Effective January 1, 2008, Florida’s testing service for administering funeral professionals license examinations has changed its name from Promissor to Pearson VUE. Pearson VUE previously acquired Promissor and had continued to operate the company under the acquired name. All telephone numbers and test center locations will remain the same.

THE REDUCTION OF FINGERPRINTING FEES
Effective October 1, 2007, the fee for the Florida Department of Law Enforcement to process fingerprints and conduct criminal background checks was reduced from $62.00 to $57.25. The fee reduction was due to the Federal Bureau of Investigation (FBI) reducing its charges to process fingerprints from Florida.

MONUMENT ESTABLISHMENTS AND PROFESSIONALS – ARE YOU LICENSED?
Effective October 1, 2005, Chapter 497, Part V, was amended to distinguish between Monument Builders and Monument Retailers. All licensed monument establishments were made Monument Retailers, and a new application was required to become a Monument Builder. The industry was allowed time to adjust to the new law and become voluntarily licensed. NOTICE: The Division’s field examiners have begun inspecting monument establishments and employees to determine compliance with the current law.

ESTABLISHMENTS SELLING PRENEED CONTRACTS - 2008 LICENSE RENEWAL
Establishments licensed to sell preneed contracts (license types 3700 and 3709) will receive their annual renewal notice beginning April 1, 2008. The notice is for renewal of the primary establishment and any branch operations selling preneed contracts. Licenses that are renewed by June 30, 2008, will be issued effective July 1, 2008.

CONTINUING EDUCATION PROVIDERS – COURSE ROSTERS
The Division uses the DICE electronic data tracking system to manage continuing education data. This system has been enhanced to allow course providers to enter course completion information directly into DICE, from course rosters, by using the attendee’s social security number or FCCS license number.
