LICENSED ENTITIES – A CHANGE IN BUSINESS LOCATION, OWNERSHIP OR A SUBSTANTIAL MODIFICATION TO THE PHYSICAL PREMISES OF A LICENSED ENTITY

A change of actual physical location, a change in ownership or a substantial modification to the physical premises of a licensed funeral establishment, cinerator facility, direct disposal establishment or centralized embalming facility requires Board approval. The licensed entity is required to submit the change for Board approval using the appropriate entity license type application form.

MONTHLY REPORT OF BODIES HANDLED, EMBALMED, ETC -- NEW FORMS RELEASED
Oct. 1, 2005 -- New forms are available on the website of the Division of Funeral, Cemetery & Consumer Services, for use in making monthly reports of bodies embalmed, direct dispositions, funerals, etc. To print copies of the forms, on the Division’s main webpage, click on FORMS, then click on the left-hand column, then scroll down to “Monthly report.” The reports are in PDF format and may be printed on a printer connected to your home or business computer. The Division of Funeral, Cemetery & Consumer Services will continue to accept monthly reports on the old DBPR forms through Dec. 2005. After that forms filed on the old DBPR forms will be returned to be re-done.

USE OF DBPR APPLICATION FORMS
Oct. 1, 2005 – Applicants for licenses should begin using the new application and other forms posted on the Division’s website, as soon as they are posted on that website. However, all applications for licenses, filed on or before 11-1-05, using DBPR forms, will be accepted and processed by the new Division, for presentation to the Board. After 11-1-05, applications filed on DBPR forms, will be returned to applicants to be re-done on the new forms available on the Division webpage.

FLORIDA LAW & RULES EXAM - WHAT LAW TO STUDY
Oct. 1, 2005 -- Applicants studying for the Florida Law & Rules Exam, should continue to use the Study Guide that was distributed by DBPR prior to 10-1-05. The Florida Law & Rules Exam will continue to be based on that DBPR study guide until a new study guide is approved by the new Board and posted on the website of the Division of Funeral, Cemetery & Consumer Services. Any such new study guide will only be applicable to applicants who apply for license after the new study guide is posted on the website.

CFO GALLAGHER APPOINTS BOARD CHAIR AND VICE-CHAIR
Sept. 30, 2005 -- CFO Tom Gallagher has appointed Mr. Gregg Brudnicki as Chairperson, and Mr. Jody Brandenburg as Vice-Chair, of the new Board of Funeral, Cemetery, and Consumer Services. They will serve a term of one year. The responsibility for appointing the Chair and Vice Chair is placed with the CFO, by section 497, 101, Florida Statutes.

NEW PRENEED SALES AGENT PROVISIONS SCHEDULED FOR NOV. 1, 2005 IMPLEMENTATION
Sept 15, 2005 -- The effective date of the new preneed sales agent license and appointment provisions, has been set for 11-1-05. The new on-line computer system needed for that system will not be ready until that date. Until 11-1-05, preneed businesses and sales agents should continue to use the forms and procedures they have used for years, to license a person to represent them as a preneed sales agent. GOVERNOR BUSH APPOINTS BOARD MEMBERS
Sept. 20, 2005 -- Gov. Bush today announced the following appointments to the Florida Board of Funeral, Cemetery, and Consumer Services:

  • Joseph Brandenburg, 62, of Jacksonville, president of Hardage-Giddens Funeral Home and Cemeteries, for a term beginning October 1, 2005 and ending September 30, 2009.
  • Gail Thomas-DeWitt, 48, of Orlando, funeral director and embalmer, Gail and Wynn's Mortuary, Inc., for a term beginning October 1, 2005 and ending September 30, 2009.
  • Justin Baxley, 26, of Belleview, vice president of operations, Hiers-Baxley Funeral Services, for a term beginning October 1, 2005 and ending September 30, 2009.
  • Greg Brudnicki, 50, of Panama City, general manager of Carriage Services, for a term beginning October 1, 2005 and ending September 30, 2009.
  • Tracy Huggins, 39, of Winter Park, funeral director and embalmer, Baldwin-Fairchild Cemeteries and Funeral Home, for a term beginning October 1, 2005 and ending September 30, 2009.
  • Peter Ballas, 88, of Tallahassee, retired Colonel for the U.S. Air Force, for a term beginning October 1, 2005 and ending September 30, 2009.
  • Catherine Zippay, 44, of Lauderdale-by-the-Sea, attorney at Hume and Johnson, P.A., for a term beginning October 1, 2005 and ending September 30, 2009.
  • Powell Helm, 51, of Bradenton, Helm Vault Service, Inc., for a term beginning October 1, 2005 and ending September 30, 2009.
